What should I bring to my appointment?

  •  Proof of U.S. Citizenship (any one document,must be original)
  • Certified birth certificate
  • Naturalization certificate (applicants 15 or under must include a certified birth certificate)
  • Consular Report of Birth Abroad or Certification of Birth
  • Previously issued, undamaged U.S. Passport
  • Certificate of Citizenship (applicants 15 or under must include a certified birth certificate)
  • Proof of Identification (any one document, must be original)
  • U.S. Driver’s License or U.S. 
  • Identification Card (if out‑of‑state ID, additional       Identification required) Current Military ID
  • Current Government ID (city, state, or federal)
  • Previously issued, undamaged U.S. Passport
  • Naturalization Certificate

What about a Passport Photo? One official passport photo taken within past six months. For additional photo requirements, visit the U.S. Department of State website. The NMB Library does not offer photo service.


What does the application process entail?


  • New passport applicants must complete form DS‑11: Application for a U.S.Passport .
  • Forms can be obtained from the U.S. Department of State website  or at the NMB Library.
  • Different forms are required  for lost or stolen passports, name changes, renewals, etc.
  • For passport renewals , follow the instructions outlined on the DS‑82: U.S. Passport Renewal Application for Eligible Individuals .
  • Applications must be completed in black ink.
  • Applications should not be signed until instructed by a passport agent.
  • All passport applicants must be present.
  • Applicants 15 and younger will need both parents present.

What about Payment?

Two separate payments are required.

  • U.S. Department of State: The U.S. Department of State accepts only checks or money orders. Checks must be made payable to the U.S. Department of State. For applicable fees, view the U.S. Department of State Fee Calculator.
  • NMB Library: Customers must pay the NMB Library a $35 non‑refundable execution fee per application. The NMB Library accepts cash, cashier checks, or money orders. Cashier checks or money orders must be made payable to the NMB Library

How long will it take until I receive my Passport?

NMB Library offers three processing options.

  • Routine processing takes 4‑6 weeks.
  • Expedited processing takes 3 weeks.
  • Hand‑carry, as appropriate.
